Nah, ill and wounded together will only kill you if you're young and awesome with more than 12 in every stat and the genius, brave and strong traits. If you're a 40-year old craven pillock then ill and wounded will just result in a scar and a strong immune system.

In ck3, a leader simulator, it makes sense it would one of the worst traits. If this was a dev-team simulator and you needed to manage a team of developers and designers, shy would be a relatively neutral or positive trait, allowing focus, while wraithful would be a terrible trait and the first person I would want off the team.
